Form 8963, Report of Health Insurance Provider Information

This form allows employers to report net premiums written for health insurance of United States health risks. The information reported will be used by the IRS to calculate the annual fee on health insurance providers.

Do not file Form 8963 in 2019 for the 2018 data year/2019 fee year.

Enacted on January 22, 2018, as part of continuing resolution legislation, H.R. 195, Division D, § 4003, suspends collection of the fee for the 2019 calendar year. Thus, health insurance issuers are not required to pay these fees for 2019.
